Dr. Farrah Tavakoli, PharmD, BCPS, DPLA completed her pharmacy education at the University of Maryland School of Pharmacy and completed her PGY1 Pharmacy Residency at Johns Hopkins Bayview Medical Center. Dr. Tavakoli is a Clinical Pharmacy Specialist for System Formulary Management for the University of Maryland Medical System (UMMS). Dr. Tavakoli oversees and leads several system-wide pharmacy specific and multidisciplinary workgroups and committees. By working closely with the clinical pharmacy managers across the system, their goal is to standardize the medication formulary and clinical pharmacy services. Dr. Tavakoli also maintains a clinical practice site providing care for adult medicine patients. She is an active member of ASHP and the Maryland Society of Health-System Pharmacists (MSHP) and currently serves as the co-chair of the MSHP Legislative Affairs committee.
